Your First Visit

New Patients

Thank you for choosing Cardiology Consultants for your cardiovascular health care. Continuation of care is important to us; therefore, we communicate regularly with your referring physician and/or primary care physician to ensure they are updated throughout your time with us.

If time allows before your visit, you will receive an automated telephone call reminding you of your appointment day and time.

We want to make your first visit with us pleasant and efficient. To achieve this, there are a few things that you can do to help us:

• Please arrive a minimum of ten (10) minutes prior to your appointment time.

• A demographic data sheet and history and physical forms will be mailed to you prior to your appointment. Please complete these before coming to our office. If you do not receive these, they are also available on our website; please print them and fill them out before coming to our office. Click here for forms. NOTE: If you have not completed these forms prior to your office visit, please arrive a minimum of 30 minutes prior to your appointment time.

• Please bring your insurance information with you and a photo ID. Click here for more information on billing and insurance.
• An accurate list of medications that you are currently taking is important in your health care. Please bring your medication bottles with you. This includes any vitamins, minerals, or other supplements you may be taking. If unable to do this, please bring an accurate written list of your current medications.

• If you are scheduled for testing, please follow the pretest instructions very carefully, especially those that relate to fasting and medication schedules. Click here for more information on tests.

When You Arrive

The 4th floor elevator opens into our lobby. The Reception Desks are located to your left and to your right. Find the name of the cardiologist you are scheduled to see and check-in at the appropriate Reception Desk.

The following forms will be given to you to read and/or sign. These forms are also online here, if you would prefer to review them and bring them with you at the time of your visit.

Your Patient Representative

Our patient representative is available to answer questions or provide any assistance that you might need. The office is conveniently located to the lobby. The patient representative can provide assistance for you in the following areas:

Assistance completing necessary forms (see above)

Directions within our office

Financial issues

Billing/insurance questions

Insurance referrals (if not done prior to the visit)

Your Appointment Time

A medical assistant will call your name and escort you to the appropriate location for your visit. The medical assistant will weigh you, ask you to change into a gown, check your blood pressure and obtain an EKG before the doctor sees you. Your history will be reviewed with you. Depending on the problem and at your convenience, we may ask you to stay longer for further testing in an effort to accurately diagnose your complaint.

As you are aware, there are times when emergencies arise and your cardiologist may be needed at the hospital. We make every effort possible to reschedule your appointment to make this as convenient to your schedule as possible.
